After reviewing the Technology Acceptable Use Policy (linked below), please complete the questions on the following page. If you have more than one student in high school, each student will need a parent to complete the consent form.

Each year, the Acceptable Use Policy (AUP) must be reviewed, the parental consent form completed and the Technology Usage Fee paid in order for a new student to receive a school-issued Chromebook or for returning students to continue service on an existing device for the next school year.
